POSITION DESCRIPTION: To provide direct patient care and activities which build and maintain the outstanding outcomes of the therapy department.

ESSENTIAL JOB FUNCTIONS:

Provides treatment programs to maximize speech, language, cognitive, and swallowing abilities within each patient’s structural or functional limitations; including, but not limited to: evaluates and treats patients, communicates with families, physicians and other interdisciplinary team , documents services in the medical records. Provides patient treatments, which meet patient needs, and uses current treatment knowledge in accordance with facility clinical procedures. This includes but is not limited to:

Performing accurate, comprehensive evaluations.

Developing effective plan of treatment and obtaining appropriate approvals from referring physician.

Utilizing appropriate evaluation tools and completing accurate evaluations.

Communicating with supervisor and other health team regarding patient progress, problems, and plans.

Instructing patients’ families or nursing staff in specified follow through programs.

Interviewing patients and families regarding previous level of functioning and life style and current and future expectations.

Demonstrating knowledge of appropriate resources for problem-solving and clinical best practice.

Evaluating patients within facility-specific time frame.

Supervising Clinical Fellow and other SLPs in direct patient care and patient related activities.

Writes accurate, complete, and clear documentation in accordance with regulatory, licensing, payer, and accrediting requirements. This includes but is not limited to:

  • Recording Resident Needs Reviews, evaluations, daily treatment notes, progress notes, and discharge summaries in accordance with facility procedures.
  • Recording treatment charges and insuring that documentation is consistent with billing data.
  • Writing specific, objective documentation, which is outcome oriented and details the patient’s needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals.
  • Managing Medicare Medical Review and Denials process in conjunction with Director of Rehabilitation, the facility team, and therapy staff.
  • Acquiring and documenting consents/approvals for treatment when necessary.

Consistently demonstrates facility’s interdisciplinary approach to patient care. This includes but is not limited to:

  • Integrating findings of other of the interdisciplinary team in treatment assessment and recommendations.
  • Integrating input from patient and family in treatment assessment and recommendations.
  • Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions.
  • Collaborating with and informing facility staff in areas of patient progress.
  • Educating of the team in areas of expertise.
  • Working with facility to teach documentation practices, which assist the therapy team.
  • Communicating effectively with discharge planners.

Contributes to professional development of self and others. This includes but is not limited to:

  • Utilizing continuing education assistance to achieve targeted quality improvements.
  • Interacting with peer therapist at local acute hospitals and other environments, if appropriate.
  • Presenting effectively at weekly rehab team conferences, facility in-services, and professional conferences.
  • Maintaining current body of knowledge in areas of clinical expertise through seminars, professional journals, and peers.
  • Meeting continuing education requirements of regulatory agencies.

Aids in building and maintaining a positive facility culture as a professional member of the staff. This includes but is not limited to:

  • Consistently demonstrating positive core values in interacting with co-workers, patients and families, with all other persons involved with the patient’s care, and the general public.
  • Consistently demonstrating professional standards.
  • Contributing to a positive work team by sharing information, using problem-solving methods and accepting new ideas, criticism, or advice from others.

  • Using proper body mechanics during treatments of patients and during activities.

QUALIFICATIONS:

Skills, Knowledge and Abilities:

  • Knowledge in Speech-Language Pathology evaluation and treatment as they relate to medically based speech, language, and swallowing disorders.
  • Ability to supervise and direct assistants and aides. Knowledge of medical terminology.
  • Ability to work with all types and levels of staff and patients.
  • Excellent written and verbal communication skills.

Education:

Master’s Degree in Speech-Language Pathology from an accredited SLP program

Experience:

2-3 years experience in a skilled nursing facility rehab environment or equivalent preferred. Rehab experience required.

Physical Requirements:

For Direct Patient Care:

  • Speech-Language Pathologist will spend approximately 60-80% of time stooping, and reaching while working with therapy patients.
  • Must be able to perform R, if required.

For Activities:

Speech-Language Pathologist will spend approximately 5-10% of time sitting at desk or terminal entering data.

License/Certification:

  • Speech-Language Pathologist Certificate of Clinical Competence.
  • Current R certification, if required.

Omni Wound Physicians is actively hiring Mobile Wound Care Physicians (MDs/DOs) across California to deliver advanced, in-person wound care services. This is a field-based, procedural physician role serving patients in skilled nursing facilities (SNFs), long-term care centers, and private residences.

This opportunity is ideal for physicians seeking autonomy, flexibility, and purpose-driven care delivery in a mobile environment. Both direct hire (W-2) and independent contractor (1099) models are available.

We are currently hiring in:

  • Southern CA: Los Angeles, Orange County, San Diego, Riverside, San Bernardino, Santa Barbara, Ojai, Oxnard, Camarillo

Schedule & Shift Options

  • Minimum Scheduling Requirement: At least two shifts per work week.

  • Shifts: Typically between 7:00 AM – 7:00 PM local time (subject to change based on business needs).

  • Full-Time: Minimum 30+ hours per week (includes full benefits package).

  • Part-Time: Minimum 2 shifts per week (paid sick time accrual available).

  • On-Call & Weekend Coverage: Some on-call or weekend coverage may be required, but team will typically be notified in advance to ensure personal commitments are not interrupted whenever possible.


Pay Transparency

  • Employment Pay Range: $245,000 – $525,000 annually

Final compensation is based on provider classification, regional coverage, experience, and productivity. This role may also include incentive or bonus eligibility depending on assignment type.

Core Duties and Responsibilities

  • Deliver bedside wound care to patients in SNFs, LTCs, and homes

  • Perform wound care procedures: sharp debridement, NPWT, biopsies, gastrostomy care

  • Diagnose and manage complex wounds (diabetic, venous, arterial, pressure, post-op)

  • Secure and document informed patient consent prior to treatment

  • Educate patients and caregivers on wound management and prevention

  • Document visits in real time using OWP’s HIPAA-compliant EHR system

  • Collaborate with SNF staff, interdisciplinary teams, and internal coordinators

  • Participate in clinical quality improvement and workflow optimization

  • Perform other reasonable duties as assigned in of organizational goals

Qualifications & Experience

Required:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree

  • Active, unrestricted medical license in California

  • Board certification in a relevant specialty (Surgery preferred; OB/GYN, Internal Medicine, or Family Medicine also accepted)

  • Minimum of 3–6 years of clinical practice in wound care, geriatrics, internal medicine, emergency medicine, or related specialties

  • Basic Life (BLS) certification

  • Reliable transportation and a valid California driver’s license

Preferred:

  • Wound Care Certification (CWSP, CWS) or ability to obtain within 1 month

  • DEA registration for prescriptive authority (preferred but not required)

  • Experience in skilled nursing, long-term care, or home health settings

  • Proficiency with EHR systems and workflow efficiency tools


Physical and Travel Requirements

  • Able to walk, stand, bend, and perform bedside procedures for extended periods

  • Able to lift up to 25 pounds (equipment, supplies)

  • Daily driving required (up to 70 miles/day)

  • Must have auto insurance meeting California minimum liability requirements

  • Must enter SNFs or homes that may lack ADA access or elevator service

  • Must document care using mobile technology (tablet, laptop)

California Compliance

  • Full Practice Authority: MDs and DOs may independently treat and prescribe

  • CME Requirement: 50 hours every two years, including courses on opioids, pain management, infection control, and implicit bias

  • Controlled Substances: DEA + CA Controlled Substance Registration required (if prescribing Schedule II–V medications)

  • License Renewal: Biennial via Medical Board of California

Principal Responsibilities

The Physical Therapist performs treatments and exercises to help patients recover from illnesses and injuries. Their main duties include using various treatment techniques to develop personalized treatment plans, prescribing medications to relieve pain and increase mobility and teaching patients exercise techniques to improve their health.

This position reports to the Director of Rehabilitation Services.

· Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.

· Diagnose movement dysfunction by listening to patients and observing them as they move.

· Develop individualized treatment plan

· Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing

· Maintain patient record by keeping track of goals

· Utilize standardized assessments and evidence-based practice to clinical interventions.

· Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.

· Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.

· Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.

· Effectively communicates with supervisor and other health team regarding patient progress, barriers, and treatment plans.

· Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.

· Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.

· Participate in facility and company required trainings, in-services and conferences.

· Able to supervise and physical therapist assistants; rehabilitation aides and/or students.

· Adherence to Company required policy and procedures and code of conduct.

· Active State Licensure
